I (NAME OF REPRESENTATIVE, TITLE) attest on behalf of (NAME OF CUD) that a failure to purchase the attached list of materials would endanger the timely construction of a broadband infrastructure.
Attached, I have included the following:
- Spreadsheet identifying all materials by quantity, manufacturer, and Stock Keeping Unit (SKU) or other unique code and description of the component.
- A memo summarizing the results of an RFP/procurement process(es) and/or providing responses from a supplier showing the lead time for each category of materials with information on how that lead time impacts the construction schedule.
- RFPs or other supporting materials showing that (please check all that apply)
___ the products have limited availability in the next 6-12 months such that orders will not arrive before the expected start of construction unless purchased before the formal approval of a construction grant.
